Our Home Care Services include:

We can provide professional and courteous care and customize it according to your needs. We will have a registered nurse provide the initial assessment and coordinate care with your physician and other medical practitioners to ensure that quality care is delivered.

Our staff is available 24hrs a days, 7 days a week to answer any questions.
Payment Methods: Third party Commercial Insurance, Long Term Care Insurance, Private Pay accounts, Workers Compensation and Medicaid Arrangements Available (Call for details).

Please note: All private pay accounts are required to give a 1-week deposit.

Nursing Care

A New York State Licensed RN provides skilled nursing care, prescribed treatments, administers medications and other services in accordance with the plan of care and consistent with the medical regime for each particular client. Teaches and demonstrates in so far as possible, health care techniques or procedures to the client and the client’s family.
Continuously assesses and evaluates the quality of services delivered in order to ensure the highest possible level of care and customer satisfaction. Demonstrates assertive and creative problem solving by identifying problems that affect customer satisfaction, service quality and efficiency and makes recommendations for improvement.
(Any Age can qualify for Nursing Care)

Certified Personal Care Aides
All of our Personal Care Aides are fingerprinted and cleared by the FBI. In addition, all workers are supervised by a Registered Nurse and participate in mandatory annual in-service hours, as required by the NYSDOH. Finally, all employees are covered under First-Care of New York Inc.'s comprehensive insurance policies (General Liability, Professional and Workers Comp)

Services Included as directed by the plan of care and supervised by a Registered Nurse:

Assist with care of the teeth and mouth.
Assist with care of the hair including shampooing and shaving.
Assist with ordinary care of the nails, including washing, drying, and filing with an emery board.
Assist with bathing in bed, tub or shower.
Assist with feeding.
Assist with walking, use of walker, or wheelchair.
Assist with dressing.
Assist with transfers from bed to chair or commode and back.
Prepare and serve meals according to instructions.
Change bed linen.
Dust, vacuum, and straighten the room that the patient uses.
Wash dishes, scour kitchen and bathroom sink.
Maintain general cleanliness of the patient environment.

List needed supplies.
Shop for patient in the absence of family members.
Launders patient’s clothes and linens in the absence of family members.

(Any Age Qualifies for Certified Personal Care)

Tasks allowable under special circumstances:
Assisting with the administration of medicines.
Measuring temperature, pulse, respirations and blood pressure.
Testing urine for glucose, ketones, and bacteria.
Collecting urine, stool and sputum specimens.
Preparation of a complex, modified diet.
Assisting with a prescribed exercise program.
Assisting with prescribed medical equipment, supplies and devices.
Assisting with special skin care, ostomy care and dressing changes.

(Any Age can qualify for Certified Home Health Aide services)

Homemakers
All of our Homemakers are fingerprinted and cleared by the FBI. In addition, all workers are supervised by a Registered Nurse and participate in annual mandatory in-service hours, as required by the NYSDOH. Finally, all employees are covered under First-Care of New York Inc.'s comprehensive insurance policies (General Liability, Professional and Workers Comp)

Services Include:

Prepares and serves meals according to instructions.
Changes bed linens.
Dusts, vacuums, and straightens the room the patient uses.
Washes dishes, scours the kitchen and bathroom sinks.
Maintains general cleanliness of the patient environment.
Lists needed supplies.
Shops for the patient in the absence of the family.
Launders the patient’s clothes and linens in the absence of the family.

Live-In Care
Our Personal Care Aides or Home Health Aides that provide Live-In Care are fingerprinted and cleared by the FBI. In addition, all workers are supervised by a Registered Nurse and participate in mandatory annual in-service hours, as required by the NYSDOH. Finally, all employees are covered under First-Care of New York Inc.'s comprehensive insurance policies (General Liability, Professional and Workers Comp).

A Personal Care Aide or Home Health Aide who will provide around the clock care under the supervision of a Registered Nurse, following an outlined plan of care. Services include all the responsibilities listed above for either the Personal Care Aide or the Home Heath Aide.
(Any Age can qualify for Live-in Care services)

Physical Therapist
Physical Therapy Services are provided to individuals who demonstrate the presence of a functional limitation. Physical therapist’s role is to provide skilled intervention to improve the patient’s level of functioning in self-care, mobility, safety, range of motion, or strength and to establish a safe and effective maintenance program. Examples of therapy services include assessment of needs and development of a therapy program for the patient.

  • Knowledge of pathophysiology.
  • Knowledge of PT Modalities and demonstrated competence.
  • Knowledge of infection control practices, aseptic techniques.

Occupational Therapist
The occupational therapist assists client’s acquire the skills necessary to accomplish ADLs. Occupational therapists their interventions on the client’s upper extremities and on the fine muscle skills needed to perform functional activities, such as eating or dressing. In addition to assisting clients to develop self-care skills, the therapist is involved in assessing the client’s home for safety and suggesting modifications to improve the client’s ability to function independently.

Speech-Language Therapist
Speech Therapy services are provided to individuals who have speech and language problems, including difficulties or delays in language development. Problems with speech and language may be either expressive or receptive. In addition, ST services are also helpful with patient’s who are experiencing dysphagia. The speech therapist works with patients to improve their ability to carry out independently ADLs related to communication or swallowing, i.e. talking on phone, eating or managing oral secretions.

  • Knowledge of speech/language treatment modalities.
  • Knowledge of infection control practices, aseptic techniques.

How to obtain Homecare Services?

Step 1 : Introductory - Intake
This is the very first step on the road to quality homecare. You have the option of filling out a form online, by mail or calling and speaking to one of our enrollment specialists  so we can help you with the homecare process by calling 718-741-5956. In order to seamlessly go through the process, you will need to supply the enrollment specialist with the following information: Primary Contact Information (Name, Address and Phone #), Emergency Contact, Insurance Information (Medicaid #, Medicare # or LTC Insurance Policy #), Date of Birth, and Primary Care Physician contact information.

 

Step 2 : Data Entry & Contract Selection
Once the information has been collected it is onto entry and selection. Your confidential and secure information will be transferred to our patient tracking system, where we will strategically arrange the best program possible to meet your needs, based on your insurance coverage. Our goal is to allow you to retain all of the services you are used to and enjoy quality homecare without having to make any sacrifices to other aspects of your life due to insurance coverage. Finally with your approval, we will submit your information to one the following Home Care Programs:

  • Managed Long Term Care Program
  • Long Term Home Health Care Program
  • Certified Home Health Agency

Managed Long Term Care Program, (MLTC) helps people who are chronically ill or are disabled and require long-term care services, such as home care or adult day care, which allows the patient to stay in their homes and communities as long as possible. The MLTC plan arranges and pays for a large selection of health and social services, and provides choice and flexibility in obtaining the necessary services from one place.

Long Term Home Health Care Program, (LTHHCP) is a variety of services that offer medical and non-medical care to people who have a chronic illness or disability. Long-term care helps meet health or personal needs. Most long-term care is to assist people with support services such as activities of daily living like: dressing, bathing and using the bathroom. Long-term care can be provided at home, in the community, in assisted living facilities or in nursing homes. It is important to remember that you may need long-term care at any age.

Certified Home Health Agency (CHHA) provide part-time, intermittent health care and support services to individuals who need intermediate and skilled nursing care. CHHA’s can also provide long-term nursing and home health aide services, can help clients determine the level of services they need, and can either provide or arrange for other services including physical, occupational, and speech therapy, medical supplies and equipment, social worker and nutrition services.

Step 3 : Contract Intake Form & Submission
Once we have determined the contract that will best serve you, we essentially become your advocates. We take your information and fill out all forms that the different programs require which allow us to get you the homecare services you need. We work directly with you throughout the entire process, while keeping you, the patient, up to date on any news and occurrences. We want to make sure you know what is happening at all times, we believe in communication and awareness, which is why we take the time to keep you informed.

Step 4 : Pre Approval Nurses’ Assessment
This portion of the process is where we finally get to know about you personally. A Nurse will visit your home at a date and time that is comfortable for you, and will conduct a full physical assessment. During this time, the Nurse will accumulate detailed medical history information in order to develop a Care-plan, which is given to us and serves as the blueprint for the care that you or your loved one will receive.

Final Step: Approval & Admission
At long last you are ready to receive homecare services. A coordinator will be assigned to you and will serve as your personal help-line. The coordinator will call and introduce themselves to you and provide their contact information. A Nurse from First Care of New York will visit you at your home to admit you to the agency which can take up to two and a half hours by performing a full assessment from head to toe, in addition to developing a detailed care plan for the paraprofessional to follow. The process is now complete.
