Home Health Aide
|
Position Description: The Home Health Aide is trained to provide home management, personal care and health related tasks to patients and families experiencing recent episodic or chronic health care problems. The Home Health Aide works under the supervision of an RN or LPN.
Responsibilities:
Assist with the care of the teeth and gums.
Assist with the care of hair including shampooing and shaving.
Assist with ordinary care of nails, such as washing, drying and filing with an emery board.
Assist with bathing in the bed, tub or shower.
Assist with feeding.
Assist with toileting, use of commode, bedpan, and urinal.
Assist with dressing.
Assist with walking, use of walker or wheelchair.
Assist with transfers from bed to chair or commode and back.
Prepares and serves meals according to instructions.
Changes bed linens.
Dusts, vacuums, and straightens the room the patient uses.
Washes dishes, scours the kitchen and bathroom sinks.
Maintains general cleanliness of the patient environment.
Lists needed supplies.
Shops for the patient in the absence of the family.
Launders the patient’s clothes and linens in the absence of the family.

| Personal Care Aide |
Position Description: The Personal Care Aide is prepared to assist clients and families with personal care and home management. The Personal Care Aide provides services to patients whose medical conditions are stable. The Personal Care Aide works under the direct supervision of a Registered Nurse or Licensed Practical Nurse.
Responsibilities:
Assist with care of the teeth and mouth.
Assist with care of the hair including shampooing and shaving.
Assist with ordinary care of the nails, including washing, drying, and filing with an emery board.
Assist with bathing in bed, tub or shower.
Assist with feeding.
Assist with walking, use of walker, or wheelchair.
Assist with dressing.
Assist with transfers from bed to chair or commode and back.
Prepare and serve meals according to instructions.

Dust, vacuum, and straighten the room that the patient uses.
Wash dishes, scour kitchen and bathroom sink.
Maintain general cleanliness of the patient environment.

Launders patient’s clothes and linens in the absence of family members Participate in conferences, and ongoing training as directed to do so.
Other such duties as assigned by the managing nurse.

| Registered Nurse |
Position Description: Renders professional nursing service to home care clients as assigned. Services include: nursing assessment, direct care, teaching, counseling and case finding. The Registered Nurse is responsible for identifying and coordinating needed services as dictated by the medical treatment plan, nursing assessment and the client environment. The Registered Nurse reports to the Nursing Supervisor.
Responsibilities:
Assures the safe delivery of therapeutic patient care in accordance with policies and professional nursing standards.
Performs comprehensive, accurate physical assessments of clients, and records such data on agency forms properly.
Records thorough and accurate health histories and initial nursing assessments.
Identifies client health problems accurately and comprehensively.
Communicates client’s conditions, responses and needs to medical practitioners and other professionals as needed to promote management of the client’s health needs.
Accepts medical orders by agency policy.
Provides skilled nursing care, prescribed treatments administers medications and other services in accordance with plan of care and consistent with the medical regime. Obtains specimens for ordered lab work.
Participates in performance improvement activities in compliance with policy and professional standards.
Continuously assesses and evaluates the quality of services delivered in order to ensure the highest possible level of care and customer satisfaction.
Demonstrates assertive and creative problem solving ability by identifying problems affecting customer satisfaction, service quality and efficiency and makes recommendations for improvement.
Formulates paraprofessional care plans, if assigned. Orients and instructs the paraprofessional to the care plan, reporting requirements, and agency policy.
Supervises the paraprofessional if assigned. Performs evaluation, teaching, and records such activities as required by the agency, according to policy.
Documents assessments, visits, communications, conferences, care plans and discharge planning and summaries by agency standards.
Completes admission and discharge paperwork in a timely fashion.

- Scheduling and Supervision of para-professionals
- Book Cases
- Update Schedules in the First Care System Daily Input Authorizations (New Authorizations, Post Hosp & On Hold’s that resume)
- Record Complaints and Incidents in the log book
- Rotate to be the On-Call coordinator for the weekend.
- Submit Discharge Summary Report to Office Manager \ Management staff within 24hrs of a case Discharge.
- Maintain the Stabilization and Weekend Booking White Boards
- Coordinators do not accept or refuse cases
